You may need to give medical evidence. Medical evidence provided for your ESAt must include medical practitioner diagnosis and the details of who made the diagnosis.
Medical evidence for ESAts can be prepared and collated by a medical practitioner, registered nurse, nurse practitioner or a registered Aboriginal and Torres Strait Islander health practitioner.
